Updated: Nominated Caregiver’s Form

SCA Ltd (Australia) Posted on 14 April 2017 by SCA Ltd Secretary2 March 2020

A new version of the Nominated Caregiver’s Form is now available, allowing for single- and multi-day guardianship at events.

This document contains information for Legal Guardians and Nominated Caregivers in charge of minors at SCA Ltd events, and the consent form required to be completed for a minor to attend an SCA Ltd event. Only the last page needs to be printed and completed.

The new form can be found here.
